Senior Care Service Agreement

Welcome to Sibex Senior Care Services, a healthcare support programme provided by Sibex Synergies Limited, designed to promote the wellbeing, comfort, safety, and dignity of elderly clients through coordinated, doctor-led home-based care.
1. Scope of Services
Sibex Senior Care Services may include:
• Doctor-led home medical visits (according to subscribed plan)
• Telemedicine consultations where appropriate
• Medication review and treatment guidance
• Monitoring of ongoing medical conditions
• Caregiver support and education
• Coordination of specialist referrals
• Ambulance coordination where clinically indicated
• Fast-lane hospital coordination where applicable
• Home treatment of selected acute illnesses where medically appropriate
2. Nature of Care
Sibex provides professional healthcare support with reasonable medical skill, diligence, and compassion. Our services support safe home-based care where appropriate, but do not replace hospital admission, intensive monitoring, surgery, or emergency specialist intervention when higher-level care is medically required.
3. Consent for Home-Based Care
By engaging Sibex Senior Care Services, the subscriber, client, or authorised representative consents to doctor-led home assessment, treatment, and coordinated medical support where clinically appropriate. Where home management is considered unsuitable or unsafe, Sibex may recommend urgent hospital evaluation, admission, or escalation of care.
4. Telemedicine Consent
The subscriber consents to communication and medical guidance through telephone consultations, video consultations, secure digital communication, and related follow-up coordination where appropriate. This may include medical advice, care updates, treatment guidance, and care coordination discussions.
5. Emergency Escalation and Transfer
Where Sibex clinicians determine that urgent hospital care, ambulance transfer, specialist referral, or higher-level intervention is medically necessary, Sibex may advise immediate escalation. If such advice is declined despite appropriate counselling, responsibility for risks arising from refusal may rest with the client, subscriber, or authorised representative.
Clinical safety remains Sibex’s overriding priority. Home-based treatment is provided only where medically appropriate and reasonably safe.
6. Subscriber Responsibilities
Subscribers, clients, and authorised representatives agree to:
• Provide accurate health information and relevant medical history
• Disclose current medications, allergies, and significant diagnoses
• Inform Sibex promptly of important health changes
• Provide safe and respectful access for home visits
• Cooperate reasonably with recommended care plans and referrals
• Treat Sibex personnel respectfully and professionally
7. Payment and Subscription Terms
Care plans become active upon confirmed payment and onboarding.
Please note:
• Subscription fees apply only to the selected plan period
• Unused visits or benefits may expire at the end of the plan period unless otherwise stated
• Benefits are generally non-transferable without Sibex approval
• Fees paid are generally non-refundable once service delivery, onboarding, or reserved allocation has commenced, except at Sibex’s discretion in exceptional circumstances
8. Confidentiality and Data Protection
Client information is handled confidentially and professionally in line with Sibex’s Privacy Policy, applicable Nigerian data protection standards, and reasonable clinical confidentiality obligations. Information may only be shared where necessary for authorised care coordination, emergency response, lawful reporting, or with valid consent.
9. Limitation of Liability
Sibex undertakes to provide services with reasonable professional competence, clinical judgment, and care. However, Sibex does not guarantee specific medical outcomes and shall not be liable for outcomes materially influenced by delayed presentation, incomplete disclosure of medical history, refusal of recommended escalation, non-adherence to guidance, or circumstances beyond reasonable operational control.
10. Acceptance
By proceeding with enrolment, payment, or use of Sibex Senior Care Services, the subscriber and/or authorised representative confirms that they have read, understood, and accepted this Service Agreement together with Sibex’s Terms of Service, Medical Disclaimer, and Privacy Policy.
