When your ageing parent needs more than occasional consultations, the Sibex Essential Care Plan provides regular home visits, ongoing health monitoring and coordinated clinical support to help keep their care organised and their family constantly informed.
What You Get with Essential Care
Regular Home Visits
Scheduled home visits to monitor agreed health and functional indicators, identify concerns and document findings.
Coordinating Doctor Support
Doctor-led clinical oversight, review of relevant findings and guidance on appropriate next steps.
Medication Monitoring
Monitoring of medication use and reported concerns, with clinical review or escalation when necessary.
Family Communication
Structured updates to help families understand their loved one’s care needs and important developments.
Specialist Referral Support
Guidance and coordination when specialist assessment is indicated. Specialist fees are included only where explicitly stated in the selected plan.
Defined Acute-Treatment Benefits (at home)
Eligible doctor-approved acute home-treatment credits in selected subscription plans, subject to stated limits and exclusions.
Clinical Escalation Support
When a senior’s condition requires a higher level of care, our Coordinating Doctor determines the appropriate escalation and coordinates referral to a hospital or requests ambulance intervention when clinically indicated.
Fast-Lane Hospital Access
Through our hospital collaborations, we help facilitate timely access to a hospital of your choice when escalation is clinically indicated by the Coordinating Doctor. This coordinated pathway is designed to minimise avoidable delays and make hospital attendance more seamless.
IMPORTANT: Escalation support and Fast-lane hospital access are both provided virtually through clinical guidance and coordination. The family is responsible for arranging an accompanying person for ambulance transfer, hospital attendance, and continued support throughout any admission. Sibex staff DO NOT provide physical escort or bedside attendance.

Hospital fees, ambulance charges, admission costs, medicines and investigations are not included unless expressly stated. Family members must arrange physical accompaniment and bedside support. Treatment credits and specialist allowances are subject to clinical approval and the applicable plan terms.
